We’re excited to invite all student-athletes to our CVHS Sports Physicals Night, hosted on campus as part of our Athletics Booster Fundraiser. This is a great opportunity for students to complete their sports physicals ahead of summer camps and the 2025-26 athletic seasons.
Event Details:
Date: Monday, June 23, 2025
Time: 3:00 – 7:00 PM
Location: Capistrano Valley High School – Small Gym
Cost: $60 per student-athlete
Payment: Cash, credit card, or check accepted
Open to: All CVHS students planning to participate in sports next year – including incoming 9th graders.
What to Bring:
CUSD Doctor’s Physical Clearance Form
(Must be completed and signed by a parent/guardian)All paperwork must be completed before arrival. The physician will complete the bottom portion of the Physical Clearance Form during the exam.
This event is made possible in partnership with AFC Urgent Care and supports our athletic programs through the CVHS Athletics Boosters.
Physical Clearance Forms can be uploaded and submitted after the event to Home Campus via the CVHS Athletics Website.
